The St. Johns River Rural Health Network, in partnership with the Health Planning Council of Northeast Florida, works to “ensure that quality health care is available and efficiently delivered to all persons in rural areas” within our region.

Who We Are

Florida State Statute 381.0406 established the Rural Health Networks to address rural health disparities in 1993. In the following year, the St. Johns River Rural Health Network, Inc. (SJRRHN) was formed as one of those networks to serve Northeast Florida. Twenty-eight years later, we continue to work towards rural health equity in Baker, Clay, Flagler, Nassau, Putnam, St. Johns, and Volusia counties as an active, community-based, non-profit organization.
